02 · The Turn

We reach out first

After an attorney, adjuster, or provider sends a referral, a clinical coordinator reviews it, listens first, and gathers what is needed without asking the patient to coordinate every handoff.

Once inside the Empower ecosystem, every patient works with two constants: their case manager and their provider. The case manager handles scheduling, follow-up, and every question along the way. The provider delivers their care. The same two people, start to finish.

03 · The Outcome

Care that gives life back

Connected care is designed so patients don't fall into the gaps between providers, records, and appointments. Evaluations, treatment, and follow-up move as one coordinated motion, in person or by telehealth.

The goal was never just a completed appointment. It is a veteran who gets his evening back with his kids. A worker who returns to function. A person who was seen, heard, and cared for the way they always should have been.
